  1. Assessment Capabilities
  • Can deliver post-tests and/ questionnaires
  • Should deliver evaluation forms completed by the participant.  These evaluation forms must include the course title, the date the course was completed, and the name(s) of the instructor(s).  These forms should allow learners to give feedback on the specific objectives of the course and on the effectiveness of the instructor(s) in delivering the course materials.
  • A self-study course delivered through an LMS must incorporate a post-test to determine if the student has mastered the knowledge, skills and abilities for the course.
  1.  Record-Keeping Capabilities
  1. An LMS must be able to maintain the following records for each course for at least 6 years.
  • the date and location of the course
  • the name and curriculum vitae of the instructor/presenter
  • the objectives and learning methods of the course
  • the outline of the course
  • the assessment methods used
  • the number of contact hours of the course
  • a summary of any evaluation methods used on the course
  • copies of all promotional materials used in the course
  • any evaluation of the need for the course
  • the list of licensed practitioners in attendance at the course or participating in the course, including the profession(s) in which each attendee is licensed.
  1.  Completion Certificate Requirements
  1. The following must be included on certificates awarding continuing education credit(s).
  • the provider’s name and address
  • the name of the student
  • the student’s licensed profession
  • the student’s license number
  • the course title
  • the instructor(s) name(s)
  • the date and location (city/state) of the course
  • the course type (e.g., live online or self-study courses)
  • the number of contact hours earned
  • a statement indicating that: “{name of approved provider} is recognized by the New York State Education Department’s State Board for Mental Health Practitioners as an approved provider of continuing education for Licensed Mental Health Counselors {provider approval number}” (NYSED Office of the Professions, n.d.).
